Discovery Health Place is a Jacksonville-based 501(c)(3) nonprofit organization dedicated to improving community health through prevention-focused education, wellness initiatives, and coordinated connections to care. Our approach combines evidence-informed health education, structured wellness programming, and clinical oversight to promote early intervention and support improved health outcomes for individuals and families with limited financial resources.
Founded to extend care beyond traditional clinical environments, Discovery Health Place delivers trusted programming directly within community settings. Our Annual HEALTH FAIR
Discovery Health Place’s annual Health Fair serves as a community gathering that connects residents with essential health-related services and information in one coordinated event. The fair offers on-site screenings, educational activities, and direct engagement with healthcare and wellness providers, allowing participants to receive guidance and next-step recommendations in real time. The event also supports community engagement by inviting local organizations, small businesses, and service providers to participate as vendors, creating a shared space for resource awareness, collaboration, and relationship-building. Through this combined approach, the Health Fair supports early identification of health needs, strengthens awareness of available services, and encourages continued participation in preventive and follow-up care.
Discovery Health Place delivers ongoing medical services with an emphasis on routine care, long-term condition support, and proactive health monitoring, all guided by in-house Nurse Practitioner Stacey Harden. The organization plays a key role in coordinating treatment and medication access by partnering closely with Ryan White case management programs and ADAP-approved pharmacies. Through this coordination, eligible individuals are able to obtain prescribed medications at no cost while receiving consistent medical oversight, follow-up care, and preventive support within a community-based setting.
